How do I become first insured?

Please refer to page 4 of the Summary Plan Description (SPD) which describes in detail the Basic Eligibility Rules. If you click on Welfare on the web site there is a drop down menu, from there click on Forms. The Summary Plan Description is shown on the Forms page. It can be viewed or you may also print the entire SPD or any page you choose.

 

 

Do I have deductibles for my Medical and Dental benefits?

If you click on Welfare on the web site there is a drop down menu, from there click on either the PPO Plan or the Kaiser Plan. The summary of benefits appears on each page and states any deductibles required for the plan.

 

Do I have life Insurance?

If you click on Welfare on the web site there is a drop down menu, from there click on Life Insurance. The summary of benefits appears on that page of this web siteThe Summary Plan Description is shown. It can be viewed in a pdf format. You may also print the entire SPD or any page you choose.
